Milton Teagle “Richard” Simmons was born on July 12, 1948, in New Orleans, Louisiana. He was the son of Leonard Douglas Simmons Sr. and Shirley May (née Satin) Simmons. His mother was a traveling fan dancer and later a store cosmetics saleswoman,

while his father worked in thrift stores and later became a master of ceremonies and a performer in vaudeville. Richard was raised in the French Quarter of New Orleans. His given name was Milton, but he later adopted the name Richard in honor of his uncle. Growing up, he struggled with obesity, reaching 268 pounds at his peak during adolescence. This struggle with weight would profoundly shape his future career. His first job in New Orleans was as a child, selling pralines at Leah’s Pralines.
